If you reach the eyeglasses or contact lenses plan maximum, the coverage amount will again be available to you once 24 months has passed since your first date of purchase.
For Example:
Marvin bought contact lenses starting on February 1, 2023.
Dates Marvin bought contact lenses: | How much the contact lenses cost: | The amount your plan will cover: | Notes: |
---|---|---|---|
February 1, 2023 | $120 | $120 | Fully covered |
August 1, 2023 | $120 | $120 | Fully covered |
February 1, 2024 | $120 | $110 | Partially covered: $350 maximum coverage reached. Marvin will have to pay $10 out of pocket. |
Total Amount | $360 | $350 |
Marvin reached the coverage maximum after getting his contact lenses on February 1, 2024.
That means…
Marvin will have to wait until February 1, 2025 (24 months from his first purchase on February 1, 2023) for his coverage maximum to reset.
Important!
If Marvin waits until April 15, 2025 to buy more, then his 24-month coverage time begins on April 15, 2025.
Some plan members may be able to get the $350 coverage for prescription eyeglasses or contact lenses sooner than 24 months under certain conditions.
Who is eligible? | What is covered? |
---|---|
Plan members who have diabetes or other conditions that require frequent eyeglass lens changes | $350 whenever there is a prescription change
Note: The first claim must include a prescription with the medical condition
|
Plan members who wear contact lenses and are unable to wear regular eyeglass frames due to the following:
| $350 for contact lenses every 12 months
Note: The first claim must include a prescription with the medical condition

Shop around — choosing eyeglasses is important and personal!
Eyeglasses prices can vary depending on where you purchase them, so you might get a bill for eyeglasses or contact lenses that is higher than your plan’s $350 coverage.

Talk to different providers and ask for a quote. This will help you find a provider you are most comfortable with, while being informed of the costs.
