What is covered?

100% up to $350 every 24 months
- Prescription eyeglasses and contact lenses every 24 months
- Laser eye surgery (no other vision benefit coverage allowed for 48 months after laser eye surgery)
- 100% up to $350 every 24 months up to R&C, for repairs.
What is not covered?
- Eyeglass cases
- Medical or surgical treatment
- Insurance form completion charges
Your eyeglasses/contact lenses coverage maximum will reset 24 months from your first date of purchase.
If you reach the eyeglasses or contact lenses plan maximum, the coverage amount will again be available to you once 24 months has passed since your first date of purchase.
For Example:
Marvin bought contact lenses starting on February 1, 2023.

Marvin will have to wait until February 1, 2025 (24 months from his first purchase on February 1, 2023) for his coverage maximum to reset.
Important!
If Marvin waits until April 15, 2025 to buy more, then his 24-month coverage time begins on April 15, 2025.
Some plan members may be able to get the $350 coverage for prescription eyeglasses or contact lenses sooner than 24 months under certain conditions.
