Important!

If Marvin waits until April 15, 2025 to buy more, then his 24-month coverage time begins on April 15, 2025.

Some plan members may be able to get the $350 coverage for prescription eyeglasses or contact lenses sooner than 24 months under certain conditions.

What is covered?
Plan members who have diabetes or other conditions that require frequent eyeglass lens changes
$350 whenever there is a prescription change
Note: The first claim must include a prescription with the medical condition
Plan members who wear contact lenses and are unable to wear regular eyeglass frames due to the following:
  • Visual acuity cannot be corrected to at least 20/70 in the better eye
  • Keratoconus
  • Irregular astigmatism
  • Irregular corneal curvature
  • Physical deformity
$350 for contact lenses every 12 months
