Before making an appointment or purchase, check that the provider, service or item is eligible and find out what the plan may pay. After receiving care, keep your documents and submit your claim promptly.
Before receiving care
Understand the cost before you commit
Before agreeing to a costly treatment, service or item, ask for a written estimate and submit it to GreenShield for a pre-determination. This is not a claim. It is an estimate of how much your asrTrust plan may cover before you receive the treatment, service or supply.
Dental treatment: Most dental offices can submit a pre-determination to GreenShield on your behalf before costly treatment, such as a crown or oral surgery. Ask the dental office to submit the proposed treatment plan before work begins so you know what the plan may cover.
Paramedical services: For services from providers such as massage therapists, physiotherapists and chiropractors, the plan pays 100% of the cost up to GreenShield’s reasonable and customary limit and your benefit maximum. Before visiting a new provider, ask about the fee. Then call GreenShield at 1-877-266-5494 or use its website or mobile app to find out what the plan may pay. If the provider charges more, you may wish to compare rates with another eligible provider to reduce your out-of-pocket cost.
Medical supplies: For items such as walkers or braces, ask the provider for a written estimate showing the item and its cost. Submit the estimate to GreenShield for a pre-determination before making the purchase.
Checking in advance can help you understand your coverage and avoid unexpected out-of-pocket costs.
Check that your provider is eligible
Before booking an appointment, confirm that both the provider and the service are eligible under your asrTrust plan. A provider may be qualified to offer a service, but that does not necessarily mean the service is covered. Alternatively, in rare cases the provider may have been flagged for investigation by GreenShield and claims from that provider will not be approved. Check through GreenShield’s website or mobile app, or contact GreenShield directly.
After receiving care
Submit your claims on time
Submit your claim within 12 months of the date you received the service or item. It is best to submit it promptly, while your receipts and details are easy to find.
Keep your receipts and documents
Keep copies of your receipts, claim forms, estimates and other supporting documents. Each receipt should show the provider’s name, the patient’s name, the date, the service or item, and the amount paid.
One last reminder: If you are unsure whether a provider, service or item is covered, contact GreenShield at 1-877-266-5494 before receiving the service or making the purchase. A quick check can help you avoid unexpected costs.
